Welcome to Smile And Style Dental LLC. By accessing or using our dental services, you acknowledge and agree to the following terms and conditions. If you do not agree to these terms and conditions, please do not use our services.

Patient Responsibilities
As a patient of Smile And Style, you are responsible for providing accurate and complete information about your medical history and current health status. You must inform our staff of any changes to your health status or medications. You must also follow all instructions given to you by our dental professionals and attend your scheduled appointments.

Treatment Recommendations
Our dental professionals will provide treatment recommendations based on your individual dental needs. You have the right to ask questions and receive explanations about the proposed treatment options. However, the final decision about treatment lies with you. We will not be held responsible for any consequences resulting from your decision to decline recommended treatment.

Payment
Payment is due at the time of service. We accept cash, checks, and credit/debit cards. If you have dental insurance, we will submit claims to your insurance company on your behalf. However, you are responsible for any copays, deductibles, or other fees not covered by your insurance policy.

Appointments and Cancellations
To schedule an appointment, please call our office or use our online appointment scheduling system. If you need to cancel or reschedule your appointment, please give us at least 24 hours’ notice. Failure to provide adequate notice may result in a cancellation fee.

Emergency Care
If you experience a dental emergency during our regular business hours, please call our office immediately. We will do our best to provide you with prompt care. If you experience a dental emergency outside of our regular business hours, please call our emergency phone number for further instructions.

Confidentiality
We take patient confidentiality seriously and will not disclose your personal health information without your written consent, except as required by law. We will only use your personal health information for the purposes of providing dental services to you and for billing and administrative purposes.

Complaints and Grievances
If you have any complaints or grievances about our dental services, please contact our office immediately. We will do our best to address your concerns and resolve any issues.

Limitation of Liability
In no event shall MH Smiles, its affiliates, or their respective officers, directors, employees, agents, suppliers, or licensors be liable for any damages, including, without limitation, direct, indirect, incidental, special, consequential, or punitive damages, arising out of or in connection with your use of our dental services.

Governing Law
These terms and conditions shall be governed by and construed in accordance with the laws of the state where Smile And Style is located.

Modification of Terms
We reserve the right to modify these terms and conditions at any time without prior notice. Your continued use of our dental services after any such changes constitutes your acceptance of the new terms and conditions.
